You don't have to think about how to get out of bed in the morning. That pattern of behaviors was set down a long time ago in your subconscious. You just do it. It can be the same for choosing that second piece of chocolate cake. Your subconscious may drive the desire without you consciously understanding why.
Trying to change a habit with rational thought alone creates a battle between the conscious and subconscious. If we don’t succeed at making changes with conscious willpower alone we tend to feel like a failure.

What is hypnosis?
Usually, we are focused at the conscious level and the subconscious is working in the background. In hypnosis, we simply flip that temporarily: you become focused on the subconscious and the conscious moves into the background. You are still aware consciously of your surroundings and thoughts. You’re just not focused on those surroundings or thoughts.
You can get past the busy mind that keeps you stuck and work with the subconscious that controls most of what you do.
What is hypnotherapy?
Hypnotherapy is simply the use of hypnosis to help people make changes at the subconscious level.
With hypnotherapy you can get at the subconscious roots for your behaviors. You can get the subconscious on-board with behaviors and attitudes you know consciously you want to do and be. You can practice desired changes by imagining yourself in situations using your desired behaviors.
What is meditation?
I think of meditation as hypnosis lite.
Like hypnosis and hypnotherapy it involves quieting the busy conscious mind so that you can focus inward but it does not usually include the same structured efforts for deep relaxation.

When you address issues on both the conscious and subconscious levels you more than double your chances of making change happen. You get both levels of your mind working together instead of battling each other.
